All correspondence must include the permit number and the route number and a valid email address. Submit correspondence to our office by mail, fax (703-815-1453), or in person.
Use the Permit Update Request Form for all extension, reinstatement and inspection requests.
Cancellations
- Work has not started
- Request permit cancellation in writing
Completions / Surety Refunds
- Work has been completed
- Request permit inspection using website
- Include current mailing address for all surety refunds
Extensions
- Permit is not expired and has not been previously extended
- Request permit extension in writing, stating reason(s) extension is required
- See 24VAC30 for administrative fee
Re-instatements
- Permit is expired or has been previously extended
- Request permit reinstatement in writing, stating reason(s) re-instatement is required
- See 24VAC30 for administrative fee
Revisions
- Scope of work has changed
- Complete and sign a Revision Application
Pavement Open Cut Requests
- Submit completed and signed Pavement Open Cut request (LUP-OC NOVA)
- Include a stamped self-addressed envelope, pre-paid Federal Express or UPS shipping label, or inform us that you will pick up the approved Open Cut from our office.
- Include photographs of pavement to be open cut (digital are acceptable)
- Include plans (1 copy)
- Include traffic control plans signed and sealed by a VA Professional Engineer (4 copies) or include temporary traffic control plans from the latest edition of the Virginia Work Area Protection Manual.
